The SUNYCard Implementation Team is seeking your assistance with preparing a profile of
your current student ID card program and addressing other areas which may be associated
with the new SUNYCard. Answers to the following questionnaire will assist us in providing
a quick response when asked to support your efforts in card-related areas. Also, the
questionnaire will help the campus and the Implementation Team to better understand the
issues you should address in considering a card program that combines multiple
applications with the a single ID card.
Involvement from the following campus departments is suggested for your campus
response:
- Registrar's Office
- Information Technologies
- Public Safety
- Business Services
- Auxiliary Services
- Library Services
- Administrative Computing
- Others, as appropriate
Prior to the initial SUNYCard Overview Meeting with your campus, please mail or fax the
